| Objectif. Reconnaitre une fistule carotido-caverneuse (FCC) devant une exophtalmie post traumatique et mettre en évidence les difficultés rencontrées dans la prise en charge de cette pathologie, véritable urgence vitale.
Observations. Il s’agissait de deux patients qui ont consulté dans notre service pour une exophtalmie post traumatique d’apparition progressive. Le premier présentait une exophtalmie unilatérale axile, indolore et pulsatile associé à un volumineux chémosis tandis que le second présentait une exophtalmie douloureuse associée à des acouphènes pulsatiles. L’angioscanner et l’artériographie réalisée en urgence ont permis de mettre en évidence la FCC. L’indication d’une embolisation transartérielle en urgence a été posée. Mais compte tenu de son coût élevé, seul le second patient a pu bénéficier tardivement du traitement.
Discussion. La FCC est une urgence vitale. Elle doit être recherchée devant toute exophtalmie post traumatique.L’embolisation transartérielle en urgencepermet d’éviter les complications graves. Cependant, le manque de plateaux techniques, l’insuffisance de spécialistes et le coût élevé du traitement estimé, à 15 000 000 FCFA dans notre contexte constituent un obstacle à la prise en charge de cette pathologie.
Objective. to recognize a carotid-cavernous fistula (FCC) in the face of post-traumatic exophthalmos and above all to highlight the difficulties encountered in the management of this pathology, which constitutes a real vital emergency.
Cases report. These were two patients who consulted our department for post-traumatic exophthalmos with progressive onset. The first had unilateral axial, painless and pulsatile unilateral exophthalmos associated with bulky chémosis while the second had painful exophthalmos associated with pulsatile tinnitus. Angioscanning and emergency arteriography revealed FCC. The indication of transarterial embolization in emergency was raised. But due to its high cost, only the second patient was able to benefit from the treatment late.
Discussion. The FCC is a vital emergency. It must be sought in the presence of any post-traumatic exophthalmos. Emergency transarterial embolization avoids serious complications. However, the lack of technical facilities, the lack of specialists and the high cost of treatment constitute an obstacle to the management of this pathology.
